Ayrton Senna de Silva (1960 – 1994) was a Brazilian racing driver who won three Formula One world championships and is regarded one of the greatest Formula One drivers of all time. He was killed during a race while leading the 1991 San Marino Grand Prix when his car crashed into a concrete barrier. On lap seven, the second lap at racing speed, Senna's car left the racing line at 190 mph. Senna's car went in a straight line off the track and struck an unprotected concrete barrier.

Upon impact the front right side of his car was pushed up and into the cockpit, striking the right frontal area of his helmet. The violence of the wheel's impact pushed his head back against the headrest, causing fatal skull fractures. A piece of suspension from the wheel detached and slammed into his head. The final injury was a jagged piece of the upright assembly had penetrated the helmet into the visor. All three of this major injuries are listed as cause of death as they occurred instantly.
